Health Quality Innovator of the Year

Clay County Medical Center (CCMC) has been honored as a Health Quality Innovator of the Year, an annual award presented by Health Quality Innovators (HQI). This prestigious award program recognizes and celebrates organizations that are using successful, evidence-based approaches to improve health care quality and outcomes.

CCMC was selected as a runner-up in the Population Health category for its efforts to improve patient care through its Transitional Care Management (TCM) program. This category recognizes organizations that work to improve the health of patients and communities through prevention, treatment, and improved access to care.

Launched in October 2023, the TCM program focuses on enhancing follow-up care within seven days of discharge, a crucial time period when patients often face challenges in transitioning from inpatient care to home. The program has shown remarkable progress in addressing these challenges.

Starting at a post-discharge follow-up rate of 86.96%, the program has improved to an impressive 97.56% — and CCMC continues to strive toward a 100% goal. By identifying key gaps in patient understanding, appointment adherence, and communication with primary care providers, CCMC’s multidisciplinary team has implemented targeted strategies such as follow-up calls, medication checks, and enhanced care coordination. Despite encountering some challenges with workflows and billing, CCMC remains dedicated to improve patient outcomes and reduce readmissions.

The winner of the population health category was Sentara Health, whose innovative partnership with The Union Mission in Norfolk, Virginia, has significantly improved health outcomes for homeless individuals. Additionally, Primeplus Senior Center was also recognized as runner-up for its successful diabetes prevention program for seniors in southeast Virginia.
