Beginning on May 19, patients of Clay County Medical Center (CCMC) and our associated primary care clinics, may receive a call from our new healthcare partner, ChartSpan. If you are contacted by ChartSpan, please know that it is not a scam or spoofing phone call.
ChartSpan is a nationwide company that provides Chronic Care Management services to Medicare patients with two or more chronic conditions. This program will give our Medicare patients access to healthcare professionals 7 days a week, 24 hours a day, 365 days a year. Patients are NOT required to enroll in this program, but it is recommended as an additional resource for all your healthcare questions and needs. “The goal of this program is to keep our Medicare patients healthier by staying in touch with each patient between medical provider appointments. Once enrolled, patients may contact ChartSpan at any time with questions or concerns regarding their health, chronic conditions, medications, or any healthcare related question,” said Austin M. Gillard, CEO of CCMC.

- Personalized care plan focused on achieving patient-guided long-term health goals.
- Help with medications, scheduling appointments, and arranging transportation.
- Answering any questions patients have about their healthcare.
- Assistance with medication refills and support for finding discounts for expensive medications.
